Narrative
On 6/29/2022 at 5:02 pm cst, in preparation for maintenance, operator visually checked the 1" vent line for product. However, the operator only checked the upstream side of the check valve, which was empty, and the operator did not check the downstream side where the pipeline still had product. Once the blind was removed, approximately 0.62 barrels of crude released to the containment area. The leak was stopped by bolting up the flanges; oil was cleaned up from the containment area. To prevent recurrence, an operator was retrained on "first line breaks".
