Narrative
At 09:52 a control room controller received a movement alarm on tank 170-4. The circulating pump was shut down and local operator was dispatched to walkout the lineup. At 11:30 a local operator confirmed a leak on 170-4 36" tank line inside of the vault containment. The flange on the pipe side of the tank valve was determined to be the source. Valve exv-46027 was isolated by the control room to stop the leak. Vault was pumped out and rinsed. Line was stripped in order to allow for maintenance to change the gasket. It was identified that the flange was not properly sealed during the initial bolt process that occurred in march 2022. Upon initial use of the 36" tank line, the leak occurred. Tank line piping cut and refitted to allow for proper seal on flange. In order to prevent recurrence, kinder morgan is in the process of updating engineering procedure cs40052426 (handling, installing, welding and testing of DOT regulated pipe) to include flange makeup tolerances. Additionally, kinder morgan terminals project managers were retrained regarding the requirement of using oq qualified contractors for these types of projects.
