Narrative
On 6/29/22 at 12:34, operations received a call informing that a 2nd party row mowing contractor had struck the 22-inch abbeville to denver pipeline with a skid steer with mower. The strike caused gouging damage and a through wall penetration of in the pipe. At 12:35, the control room responded by shutting down the upstream pump and blocking in the line with mainline valves upstream at hornsby station and downstream denver station. At 13:35 mainline valve at denver was opened and downstream pumps were started to pull product away from the leak site. At 13:59 mainline valve at denver was closed again. Approximately, 4800 barrels of crude oil was released and nearby horse creek was impacted. Company personnel and oil spill response contractors were mobilized to contain and recover released product. Company set up an incident response team that worked with all federal and state agencies to ensure response, recovery efforts and pipeline repair and restart were completed appropriately. On 6/30/22 at approximately 12:45, a plidco mechanical clamp was installed and on 7/1/22 at 19:02, the line was restarted. Supplemental report: part g updated to other outside force damage - if damage by car, truck, or other motorized vehicle/equipment not engaged in excavation. The type of vehicle involved in the accident was a cat brush cutter, model brx418 industrial. Since type of vehicle cannot be left blank, motorcycle/atv was selected as the best option. 5mph, motorcycle/atv, off road, 0 and "no" selected for questions 7 through 11 respectively at the recommendation of PHMSA aid to more clearly explain accidents of this nature caused by row mowing activities.
