Narrative
On 6/26/22 at 07:21, houston control center notified an operation tech about a release at advantage lact unit delivery. The system was shutdown and operations personnel dispatched to the station. At 08:20, operation tech arrived on site and confirmed a release of 3.8 bbls onto the ground from a sump overfill. The cause of release was determined to be a failed diaphragm on the back pressure valve at the lact unit. When the diaphragm failed it caused the relief system to activate which is piped to the sump. The sump could not keep up with the volume being relived which led to the sump to overfill. The lact unit was isolated and the ruptured diaphragm on the back pressure valve was replaced. The sump level alarm was also programmed to alarm in the remote control center which will allow the controller to take appropriate actions to empty sump and/or shut down incoming lact units in order to prevent a reoccurrence of the same or similar event to that which occurred on 6/26/22. .
