Narrative
At 3:15 pm on june 8, 2022, the mont belvieu operation first line supervisor (fls) received a call from oneok stating that there was a release occurring inside of purity station. Fls called operations control center, and they remotely isolated the inlet and outlet of the facility. Fls and technician arrived to site, accessed upwind, and isolated the system closer to the leak source using manual valves. The isolated section of pipe was allowed to bleed off until line pressure reached 0 psig, resulting in a 184.32-pound release of isobutane to atmosphere. Atmospheric corrosion was located on the bottom of piping at interface with pipe support, causing a pinhole leak. To prevent recurrence, operations removed the pipe spool and installed pressure rated blind flanges. As part of its mitigation plan, mobil pipe line will have a qualified corrosion inspector, from outside the operational area, perform a new atmospheric corrosion inspection within the storage area of operation, including pipe supports. Per our atmospheric corrosion program, we will develop a repair plan for anomalies, if any, found during the cold-eyes review. Area management will also purchase handheld mirrors to assist corrosion technicians with inspections in hard to reach areas, including to pipe to support interfaces. In addition, corrosion technician will be requalified with a skills demonstration on perform atmospheric inspections to confirm inspections are meeting our corrosion control program practices.
