Narrative
On 6/9/22, around 12:30 am, an operator was dispatched to check an excessive sump pump running alarm at colex east tank farm. After checking the sump pump, the operator made a round by the manifold and discovered a small puddle of gasoline collecting near the interface of some below-ground piping. The operator notified supervision and it was determined to keep an eye on the location until daylight for further evaluation. At 7:40 the same day, it was determined to dig up the area and locate the source of the leak, and this time it was identified the costs would exceed $50k, and then the call to the nrc was made. Concerning cpm, the leak occurred on facility piping near the manifold and that doesn't fall under the cpm installed. In saying that, there is terminal balance leak detection primarily based on the ehh on the tanks. This system is designed to pick up a much larger release. After the investigation, it was determined the gasket failed from improper alignment from the original installation date.
