On 11/28/22 at approx 23:22, tl-442 ruptured at sta 5+00 under an access road on company property in pine grove, wv. Initial notice was received from an outside party at approx 23:23. Tl-442 is a 16 in od transmission pipeline approx 0.30 mi long and transports natural gas from hastings cs to mockingbird hill cs. The maop of tl-442 is 990 psig. A pressure restriction of 960 psig was established in 2003 for tl-442 due to its connection to tl-430 at haught jct. Tl-442 was in normal operation with an op of 947 psig when the incident occurred. At approx 23:30, ops personnel called 911. Also at approx 23:30, the c-300 unit at hastings cs shutdown and the associated unit valves closed, isolating tl-442 upstream of the rupture. At approx 23:33, gas control remotely closed valve-420jj, isolating tl-442 downstream of the rupture. At approx 23:50, ops personnel arrived at the site. The rupture resulted in an approx 40 ft length of pipe opening circumferentially and longitudinally (not seam oriented) and both ends folding up and coming out of the ground; approx 23,403 mcf of gas loss; damage to the access road; and debris being blown on the right-of-way, s.R. 20, and a railroad. S.R. 20 and the railroad were temporarily closed at 23:40 to remove debris. The gas did not ignite and there were no fires, injuries, fatalities, or evacuations. It was determined on 11/29/2022 at 00:44 that the incident involved an unintentional estimated volume of gas loss of 3 mmscf or more. Initial notification (#1353645) was made to the nrc on 11/29/22 at 01:04. Follow up notification (#1353781) was made to the nrc on 11/30/22 at 11:46 to confirm the initial notification and update the current status of the tl-442, the gps coordinates, the estimated volume of unintentional gas loss, and the temporary closure (and reopening) of s.R. 20 and the railroad in order to remove debris. The estimated volume of unintentional gas loss was later confirmed to be 23,403 mcf. Tl-442 was restarted on 12/30/22 at 14:09 after completion of the following remediation: approx 485 ft of station piping at hastings cs associated with tl-442 was removed and replaced along with valve-425d to resume the flow of gas between hastings cs and mockingbird hill cs through tl-597 and into tl-430 while the remediation of tl-442 was completed. Both ends of the pipe were cut at the rupture on tl-442, upstream and downstream of the hinged areas where the pipe folded up and came out of the ground. The two pieces of pipe that were cut-out at the rupture were sent to kiefner for metallurgical analysis and replacement pipe was installed. The metallurgical analysis report from kiefner determined that external corrosion where disbonded polyethylene coating was shielding cathodic protection was the apparent cause of the failure. Pipe was cut-out at selected locations to provide access to the inside of tl-442 for wireline inspections. Wirelines using cal and mfl tools were conducted to assess the integrity of tl-442 and replacement pipe was installed at the access locations. Initial verification digs were completed on either side of the rupture to assist in validating the accuracy of the data from the wirelines. Based on the data from the wirelines, the results from the initial verification digs, and the evaluation of anomalies as described below, approx 950 ft of tl-442 was replaced. This involved replacing two sections of tl-442, from valve-425d to the south side of fishing creek and from the north side of s.R. 20 to mockingbird hill jct. The anomalies located in these two sections were evaluated as the pipe was excavated prior to removal and further verified the accuracy of the data from the wirelines. Approx 120 ft of tl-442 located in a casing under the railroad and s.R. 20 was removed and replacement pipe was installed. An anomaly located on the cased pipe that was removed was evaluated and further verified the accuracy of the data from the wirelines.
