It is centric's opinion that the primary root cause of the incident was equipment failure (hydraulic squeezers). Through investigation it was reported that something malfunctioned in the hydraulic pump, causing the jaws of the squeezers to release and allow gas to enter the hole. Broadhead has requested maintenance records from equipment supplier gajeske, inc. But these records have not yet been provided by gajeske, inc. Centric was initially informed that no clamps were provided with the rental. After follow-up questioning centric received the following from broadheads lawyer "the locking devices were provided but they were not used." leading US to identify this as a contributing factor. Other contributing factors include that the contractors were not wearing appropriate ppe (in contravention to their contract with centric), and hazard recognition was inadequate by the contractor staff on scene.
