Narrative
On 7/22/22 at approximately 10:10, a producer contacted energy transfer operations personnel to report a loss of pressure and well shut in bienville parrish. The 2 wells reported to be shut in were both inlets into the energy transfer pipeline referenced as 20101088 tristate 9047 chk weyerhaeuser 15-1. Operations personnel were mobilized and enroute to shut manual valves to isolate the segment of pipeline. The upstream and downstream valves were shut in and energy transfer personnel located a pipeline rupture on the weyerhaeuser 15-1. Once isolated, the pipeline was allowed to blowdown at the rupture site. A segment of pipe was located outside of the rupture area and was preserved to send for metallurgical analysis. Reports were made to the la state police on 722/22 at 10:22 and la dnr on 7/22/22 at 11:46. Nrc notifications were made (1342244 & 48-hr follow up 1342376). Metallurgical analysis is being completed on the sample recovered from the rupture site. The segment of pipe affected has been replaced and was placed back in service on 8/5/22 at 18:00. Metallurgical analysis indicated internal environmental cracking as the cause likely due to interaction between corrosion inhibitor and components of the gas stream. Since the time of the failure the pipeline was subjected to a hydrostatic pressure strength and spike test.
