The cause of the incident was unintended valve movement. The fuel gas blowdown valve on the turbine opened while the supply was also open. Blowdown valve is a 2-inch plug. No alarms were associated with the event. The investigation performed determined one of the contributing factors was the lack of an alarm when the blowdown valve was out of position such that the operator would not have been able to recognize the valve was out of position. Alarm installation scheduled already. Fuel gas logic and alarm installation has been completed.
