Narrative
On september 9, 2022, at approximately 11:23 hours consumers energy received notification of a natural gas pipeline damage on hall road (west of avon lane), clinton township (macomb county), michigan. A gas service worker (gsw) was dispatched and arrived onsite at approximately 11:51 hours. The gsw confirmed the damage which was later determined to be a 2-inch stub (off a 6-inch high-pressure pipeline) struck by a third-party contractor installing drain tiles. A distribution crew arrived onsite at approximately 12:09 hours and proceeded to excavate two remote holes to the east and west of the damage location. While attempting to squeeze the pipeline on the west side of the damage, the pipeline cracked and required excavation at a third location. A mueller control fitting was installed west of the damage to contain the blowing gas. The pipeline was a one-way feed and resulted in a loss of service to 20 customers. No fatalities or injuries occurred because of the incident. The damaged segment of the pipeline was cut out and replaced with approximately 6-feet of 6-inch high-pressure pipeline. Repairs were completed and gas service was restored on september 10, 2022, at approximately 11:35 hours. Service restoration to impacted customers began after repairs to the pipeline were completed. An incident investigation determined the a 2" natural gas stub was documented on the record but was not marked by the staking employee resulting in a natural gas pipeline damage.
