Narrative
On january 31st, 2022, brinker compressor station (cs) experienced an unintentional release of natural gas (material) into atmosphere. Brinker is a transmission cs that is operated and maintained by columbia gas transmission, llc (company), a tc energy entity. Brinker cs is located in columbiana county, ohio. At 17:45 est, a company employee received a notification from the public of a fire at brinker cs. Company operations personnel arrived on-site at 18:00 est and the fire on the dehydration unit was confirmed. After an initial assessment of the situation and taking all safety precautions, personnel isolated the fuel gas supply to the dehydration system at 18:35 est and the fire was extinguished. The preliminary material loss calculation was determined to be 44 mcf. On february 4, 2022, after the damage caused by the fire was fully evaluated and assessed, it was determined that the cost of repairs may exceed $122,000 so the event was promptly reported to the national response center (nrc). An investigation team was formed to determine the probable cause of the fire on the dehydration unit. The initial nrc report no.1328093 and the nrc 48-hour report no.1328149 have been received on february 4th and february 5th respectively. An investigation was completed, and the following casual factors (cf) were identified: 1- internal mesh of the contractor prevented wet glycol return valve for closing. 2- due to the glycol flooding the reboiler and surge tank, gas pressure did not reach pressure safety valve (psv)-100 that was located on reboiler, therefore psv-100 did not activate to release gas pressure. The following recommendations were issued in order to avoid future re-occurrence from taking place at this cs. 1- create preventive maintenance and monitoring plan specifically for the brinker contact tower with increased internal inspection 2- the tower will be replaced. 3- install high level alarm for the reboiler and surge tank that alarms to the station control panel and high-high switch to drain the reboiler the project at brinker cs was completed and dehy was placed into service on december 2, 2022,
