The use of mechanized equipment in the tolerance zone & excavator failed to support and protect facilities. Water company contractor was on a site to install water service to a customer. Tapping what they believed was the water main, was instead an 8" high pressure gas main. The water contractor stopped when gas was released and reported a gas leak.
