Incident summary on monday, january 24, 2022, frank coluccio construction ("fcc") was performing a sewer installation on a portion of waiakamilo road in honolulu, hawaii, with an auger. At approximately 9:02 a.M., on the portion of waiakamilo road between hart street and kalani street, the auger struck a hawai'I gas ("hg") underground 8-inch pipeline ("subject segment"). The auger caused an approximately 3-inch long fracture along the top of the pipeline. The subject segment began to leak gas. Hg's first responder arrived on site at approximately 9:15 a.M. And confirmed the damage to the subject segment. Due to the magnitude of the leak, hg called the honolulu fire department ("hfd") at approximately 10:19 a.M. To assist in securing the roadway and to help monitor the hot zone. Hg's maintenance crew arrived on site at approximately 10:30 a.M. And began assessing repairs. At approximately 11:15 a.M., hfd began evacuating nearby commercial buildings. Hg was able to stop the flow of gas to the subject segment at approximately 5:35 p.M., january 24, 2022. Final repairs were completed at approximately 11:00 p.M., january 24, 2022. Hawai'I gas involvement prior to incident hg installed the subject segment in 1940. The subject segment is an 8-inch bare steel pipe. Hg maps and records contain no indication that the subject segment was repaired, altered or excavated since its installation. On monday, january 3, 2022, fcc submitted one call ticket number 22000022 to the hawaii one call center indicating that fcc would be excavating "on waiakamilo road between dillingham boulevard and hart street (ewa side of road)" ("excavation area"). In response to ticket no. 22000022, hg reviewed gis maps of the excavation area and determined that an underground main line was located within that area. On january 10, 2022, hg personnel attempted to perform toning on the underground main line in the excavation area but noted that the location of the underground main line had already been marked by another party in yellow paint. Please contact hg for additional narrative detail. Also, please note that this investigation is still ongoing. Update 6/10/2022 upon completion of the investigation by hg, it was determined that toning of the subject segment was performed inadequately by hg. As indicated in the "incident summary" section above, the underground main line had already been marked by another party in yellow paint and hg failed to confirm the markings were correct when responding to ticket no. 22000022. As a result of the investigation, hg has incorporated the following mitigative actions into its toning operations as of may 2022: 1. Revise "hg one call pipe location guidelines" document. A. All painted utility markings by hg will have a unique hg identifier. B. All existing utility markings (whether by hg or 3rd party) shall be reverified in response to an 811 ticket. 2. Update training requirements for all hg employees who conduct utility locations. A. Initial training requirements (oq & "hg one call pipe location guidelines" document review). B. Refresher training requirements (pipe location refresher training to be provided in 2022 and every 3 years after or when probable cause is determined by hg management). C. Documentation retention requirements. Hg is confident the mitigative actions listed above will help to further reduce toning related incidents going forward.
