Narrative
Operator personnel identified a cracked multi-port valve associated with a thermal relief valve spraying diesel fuel. Pipeline was shut down, a new valve was installed, and pipeline restarted. Impacted gravel and soil was removed from the site. The cause of the failed multi-port valve is pending analytical results. Following the accident, additional support/bracing was added to prevent reoccurrence. On january 17, 2022, marathon pipe line received the evaluation report from the manufacturer stating the failure was caused by fatigue which can by caused by excessive movement or vibration from an external source. Parts a, b, c, d, e new questions were answered. Part g was updated.
