Narrative
Phillips 66 measurement technician was performing pycnometer densitometer verification on the ethane meter skid for sr-92 at the clemens meter station, a compression fitting connection suddenly failed and separated on the 1-inch stainless steel tubing run for the pycnometer. Pipeline was shutdown, system isolated and repaired. The investigation team determined that the end of the tubing in the failed connection was not sufficiently crimped to prevent the tubing end from pulling out of the ferrule/nut connection. All other area tubing connections were verified for proper installation. Supplemental update 11/5/20 as per PHMSA aid: the tubing installation was part of a new construction project, thus oq did not apply. The connection failure occurred due to the installer not following the correct tubing crimping verification procedure. Phillips 66 plans to implement a secondary pre-startup verification process on larger diameter tubing for new construction installations.
