Narrative
At 10:33 am october 8th texas gas service (tgs) dispatch received a call from manuel derueda notifying them of a damage to a tgs facility. Tgs's 12" coated steel gas main was damaged by a 3rd party contractor (city of mcallen public works department). The city of mcallen was conducting road and drainage work with a road grader near the intersection of stewart rd and mile 8 in north mcallen, tx. The road grader was making deep cuts of soil to complete a new road construction project. Texas gas service facilities had 24" to 30" depth of cover in this area. The road grader struck the 12" pipeline causing a 4" x 5" triangular hole in its facilities. The city of mcallen did not make notification to the one-call center to perform this work. At 4:31 pm on october 8th texas gas service confirmed that the gas loss was over the 3000 mcf PHMSA reportable threshold. At 4:45 pm october 8th texas gas service reported the incident to the nrc (#1289317) and conducted the 48 hr update per PHMSA requirements at 9:34 am october 10th nrc (#1289424). Supplemental information 3/5/21 the city of mcallen has been contacted electronically concerning this incident. Also, after safety protocols allow US to (due to the pandemic), texas gas service will meet with the city of mcallen to review texas utility cody 251, specifically chapter 18, section 18.3 excavator notice to notification center, section 18.4 excavator obligation to avoid damage to underground pipelines, section 18.5 operator and excavator obligations with respect to positive response in an effort to prevent future damages. Texas gas service damage prevention group reviews all excavation damages to track and monitor repeat offenders for at-fault excavation damages, currently the city of mcallen has not yet met the repeat offender list. Texas gas service will require safe excavation meetings with the city of mcallen if additional at-fault excavation damages are experienced.
