Narrative
Contractor installing communications cable was boring perpendicular to 2 inch plastic gas main. Excavator (subcontractor) did not have a valid ticket in their name. Arkups (contract line locator) had noted that further locate activity should be performed on 2 inch plastic prior to excavation and left a sign at the location to be called before excavating in the affected area. Excavator (subcontractor) pot-holed on marks to verify that gas line was located where marks indicated, but mis-identified a different cable as gas piping. Boring head damaged pipe resulting in separation of 2 inch plastic main. Cnp repair crews were excavating in remote location to isolate system and replace damaged main when ignition occurred. Additional cnp crews were called to the site to isolate the system and shut off gas feed to the 2 inch plastic main. The investigation is ongoing.
