Narrative
While road work was being done around a 14" main by a soft dig crew in a cul-de-sac, a gas odor was detected. A crew begin drilling holes in the asphalt to pinpoint where the leak was coming from. A pinhole size leak was found in the steel gas line and was hit by a bore. The use of mechanized equipment in the tolerance zone & excavator failed to support and protect facilities.
