Narrative
Columbia gas of ohio, inc. (columbia) was notified by its scada and by the general public of a leak at the intersection of sr42 and hanley rd. Columbia found a 12" steel main line leak where a short barreled bolted coupling without pull out restraint failed. Columbia shut off the line for safety and made permanent repairs. This event resulted in a service disruption to 996 customers. Please consider this report final.

Investigation Status Details
We received numerous reports from the public at the same time low pressure alarm was received via gas controls scada system upon pressure dropping from 96 psig to 4.2 psig. The gas controller evaluated other scada indications and noticed a corresponding increase in flow, prompting them to call-out to the operations leader. The operations leader informed the gas controller they had been notified of the incident from other parties as well and that technicians were responding.
