We conducted a detailed investigation with the controllers, pump vendor, and gspm operations. The result of this analysis showed the the pump failed after deadheading on start-up causing excessive heat build-up. The vendor confirmed that the conditions where the pumps was operating should not have resulted in a dead head condition no a pressure related failure. They took the pump to their shop for a detailed analysis and could not determine a specific cause. The damaged equipment was replaced per manufacturer recommendations.
