Narrative
On june 2nd, 2019, a third party company observed and reported a leak on the pecos to crane 24" launcher to an oryx midstream services company employee at 08:40. The 1/2" nipple was stuck causing the nipple to bend out of the threads from 1/2" to 2" reducer. The cat walk was newly constructed, but the anchor points were not installed at the time. The cat walk has been removed from the area until the anchor points can be installed.
