Narrative
During normal operations, the holly energy partners control center received an lel vapor monitor alarm at hawley station at 00:22 mt via the scada system. The pipeline was immediately shut down and blocked in at 00:22 mt. A technician was dispatched to the location to investigate further. Once holly energy partners personnel arrived on-site at 01:05 mt, they were able to confirm that a piece of steel tubing had failed on hawley stations pumping unit #2. Holly energy partners personnel on location also reported that there was a release of product from the failed tubing. During a root cause investigation, it was determined that the failure was a result of high-frequency vibration.
