Approximately 4.3 bbls of crude oil were released with secondary containment from a stainless steel process cooling line on pump 105. The date and time of the release (05/17/2019 @ 16:00), is indicative of the earliest possible time the release could have started, as it was the last time the location had been visited and confirmed that no crude oil was being released. To prevent future releases, plains removed the stainless steel cooling line.The cause of the equipment failure is under investigation, and a supplemental report will be submitted pending third party analysis of the equipment. Supplemental narrative: results of third party analysis indicate that the tubing failed by fatigue crack propagation, likely caused by vibration. The fracture initiated on the od of the tubing, adjacent to the location of the ferrule.
