Narrative
On august 30, 2019 columbia responded to a large commercial customer to investigate a relieving customer-owned house line regulator. Columbia discovered that it had exceeded the normal houseline operating pressure of 45 psig. Columbia first responders investigated and discovered the house line operating pressure had reached 177.9 psig. Columbia then shut the valves feeding the station and depressurized the house lines down to 0 psig. The over pressurized house lines resulted in potential damage to 18 customer-owned regulators. Columbia rebuilt the jurisdictional regulator station and installed partial pressure relief valve. Columbia has worked with the customer to safely replace all potentially damaged customer-owned house line regulators. Columbia conducted a full scale jurisdictional regulator inspection in the field on columbia-owned regulators as well as conducted testing in the lab to try to ascertain the cause of the failure. Columbia was unable to duplicate the failure in the laboratory and believes the likely cause of the failure was debris on a sealing surface that allowed an intermittent bleed through.
