Narrative
A third-party construction company (opal construction) damaged an 1 1/4" purge riser on a 2" steel main at mill rd and peninsula blvd in hewlett while doing reconstruction to a parking lot during the milling phase of reconstruction. A markout request was issued several months earlier but paint marks were not maintained or visible. The main caught fire due to the damage. A paving machine also caught fire and sustained significant damage. Estimated time of incident is approx. 11:30 am. Estimated value (salvage value) of paving truck was $125,000. There were no injuries. The fire department was on location and evacuated a nearby shopping mall as a precaution. A field operations crew responded immediately at 11:34 am and at 12:37 pm performed a squeeze off on the main to make safe. This resulted in a supply interruption of five services. The main was repaired on 08/29/2019 at app. 20:01, gassed in at 8:15 pm. Repair was made by threading and capping 1 1/4" damaged riser. Services were restored on 8/30/2019 at 11:00 am.
