Narrative
A commercial customer requested that tgs relocate a 125' section of a 12" coated steel pipeline to accommodate the customer's new building construction. The customer's new building plan required a footer in close proximity to the existing pipeline. On 8-28-19, after isolation was complete, work began. The entire 125' of 12" coated steel main was laid in a new ditch and getting prepared for two 12" steel tie-ins to complete the relocation. Later in the day, upon detecting natural gas bleeding through a 12" isolation valve into the isolated pipeline, the texas gas service inspector ordered work to stop until a thread-o-ring (tor) could be installed to pull gas from the isolated piping where hot work was going to be performed. While the tor fitting was being obtained, the contractor's foreman directed his welder to attempt to ignite the gas in the north trench to test for gas. The spark traveled down the pipe to the south trench and ignited the gas there causing an injury to the employee. The inspectors, foreman and welder were disqualified and required to retrain and be re-evaluated prior to any return to work. In addition to the retrain and re-evaluation of the affected employees, texas gas service will be performing a safety training covering the importance of following construction and safety protocols when performing any construction work for texas gas service. Texas gas service at 21:45 (8/28/19) was notified that the injury (burns) to the employee would cause inpatient hospitalization which was the confirmed discovery time this is a reportable incident. The employee was released from the hospital on 8/29/19. The 48 hrs. Phmsa notice requirement was called in on 8/30/19 @ 11:01 am (nrc#1256868). Note: relocation was for a new commercial customer (car dealership that did not have existing address). The actual incident location is across the street from the 10000 research blvd address on the report which the lat / long coordinates point to.
