Contractor, cougar contracting llc, damaged peoples gas' (pgs') 8" steel high pressure main during excavation. Contractor failed to obtain a locate request for the excavation time frame. Last locate request had been obtained 60 days prior and was expired. To make the repair, a style 220 reinforcing sleeve was welded over the damage location to stop the flow of gas. Pgs then applied an f1 coating to the area then the sleeve was x-rayed. Pgs then conducted a mag-particle fillet weld test and an ultrasonic test.

Narrative
Contractor, cougar contracting llc, damaged peoples gas' (pgs') 8" steel high pressure main during excavation. Contractor failed to obtain a locate request for the excavation time frame. Last locate request had been obtained 60 days prior and was expired. To make the repair, a style 220 reinforcing sleeve was welded over the damage location to stop the flow of gas.
