Narrative
Natural gas leaking from an underground mechanical service tee ignited causing fire damage to a warehouse exceeding $50,000. Contractor pipeline installation records relating to the customer-owned service pipelines impacted by the industrial parkway incident are attached to this report. These records were provided to deo from the attorney for the property owner's insurer. The material testing protocols and associated letter regarding an evidence examination scheduled for friday, april 26, 2019 at 10:00 a.M. In connection with the incident are attached to this report. On april 25th, 2019 deo held a meeting to correct originally reported timeline discrepancies utilizing fleet gps reports and phone records. On april 29th, 2019 deo met with the investigating puco inspector to present the amended timeline report and discussed meeting results for corrective actions. The amended timeline report and meeting notes are attached to this report. On april 26th, 2019 material testing was completed in accordance with the provided testing protocols. Representatives from the puco and deo were in attendance. The material testing report was completed on may 17th, 2019 and is attached to this report. In accordance with 192.617, dominion energy ohio will utilize existing dimp processes to analyze the results of the independent 3rd party material testing report along with past mechanical fitting failure reporting data to make determinations on any necessary additional measures needed to minimize the possibility of a recurrence.
