Narrative
On september 25th at 14:12, an enterprise technician was notified by a third-party terminal supervisor that the pig trap door on the p114 pig trap was leaking diesel. Upon arrival at 14:25, the technician found the diesel was leaking into a secondary containment and a minimal amount of product had leaked into the secondary containment area. It is estimated that 10 gallons of diesel was released into the secondary containment. The failed o-ring was from the original installation in 2009 and showed normal operational wear and tear. The pig trap was isolated, the sealing surface was cleaned, and o-ring was replaced. Pig trap was available for normal operations at 15:30. Our plan to prevent reoccurrence is replacing a new o-ring with a suppliers suggested expiration date of 2030 and will be monitoring the new o-ring for future leaks.
