Narrative
At approximately 18:05 on 6/2/2018 local operations personnel discovered a release of approximately 40 gallons of crude oil at the rotation manifold. It was discovered that the body of a pressure gauge on a 3/4" pressure relief line had failed. The release was stopped by isolating the pressure relief line by closing upstream and downstream 3/4" valves. Additional personnel were dispatched to assist with cleanup. All free product was picked up, piping wiped clean and contaminated soil/gravel removed for on-site remediation. The gauge was replaced and the pressure relief line was returned to normal service. Inspection of the failed gauge concluded that the body of the gauge had failed on the backside of the gauge due a suspected manufacturing defect.
