Narrative
On 6/1/2018, usld was observed in containment at pasadena terminal's gp manifold. Upon investigation, it was discovered that the thermal pressure relief valve was popping off to relief thermal pressure but failed to seat properly. As the product filled the sump, the pump to remove product failed to engage which caused product to overflow unto containment. Product was removed via vacuum truck and the thermal relief valve replaced. Corrective actions: all discharge from relief valves are being piped directly into a transmix tank to prevent this from reoccurring.
