Narrative
On 04/12/2018 at 9:00 am, summit's operation personnel were performing routine checks at epping station. A summit operator discovered discoloration of the valve stem for the little muddy pipeline 10" valve. After further investigation, the operator determined the valve had developed a slow leak from the valve stem. The operator reported the leak to local operations management and summit's control center. The valve was greased as a temporary measure to eliminate the leak. Snow was covering the ground at the time of the leak. A vacuum truck was dispatched to assist in snow removal and cleanup. Remediation of the leak determined approximately 32 gallons of crude oil were released. The incident investigation revealed the packing on the 10" valve had failed. A new 10" valve was installed as permanent corrective action.
