Narrative
On april 10, 2018, a third-party contractor struck a 6-inch gas steel distribution line while performing trenching for a new sewer line. This resulted in an escape of gas. Crew responded and was able to control the gas through closing a valve. There were no injuries that resulted from this incident. Investigation showed that prior to the incident, the third-party contractor had a USA ticket and the line was properly marked by the utility owner. At the time of the incident, the third-party contractor did have a valid USA ticket for the excavation activities performed. In addition, the third-party contractor failed to pot hole the facilities by hand. This incident was reported to DOT and cpuc due to estimated damage amount exceeding $50,000.
