Narrative
On september 13, 2017, contract crews were on-site performing a project to remove a casing. While drilling through the casing, the contractor drilled a hole into the pipe, resulting in a release of liquid natural gasoline. Approximately six barrels were released into the excavation trench. All product was immediately cleaned up and repairs were made. The cause of this release was incorrect operations. As a result of this release, future casing removal projects were immediately suspended until procedures were improved. 2/8/18 update: part c, question 3a. - corrected the nominal diameter to 8 inches. Part c, question 6 - changed the type of accident involved from "other" to "leak".
