A stainless-steel tubing, providing thermal relief around an isolation valve on surge relief piping, failed at a connection. The tubing has been removed from service, and will be sent to a lab for analysis. Update - the failure occurred due to high cycle fatigue. Cracking initiated in a groove on the outside diameter of the tubing, created by the compression fitting back ferrule biting into the tubing wall.
