A 4-inch plastic main with one-way feed was ruptured by a contract crew while installing a power pole. The escaping gas ignited causing extensive damages to the contractor's vehicle. Notification to the one-call center was made where the main was located, however, the pipe itself was mis-located by 24 inches, 6 inches outside the state defined tolerance zone. Additional nrc report number 1186139 was created for the same event after a 48 hour supplemental call was placed with the nrc.
