Narrative
P/l # 2017-0079 southwest gas corporation (swg) responded to a report of a vehicle striking a district regulator station at the northeast corner of 75th avenue and buckeye road in phoenix. Swg was notified of the incident on may 5, 2017 at 01:20 by the fire department. Swg crews responded and controlled the flow of gas by utilizing one valve resulting in an outage of six commercial customers. Subsequent repairs were made and the facility was placed back in service on may 6, 2017 at 07:58. There were no injuries or fatalities as a result of the incident. Swg notified the national response center (nrc) on may 5, 2017 at 14:11 after determining the significance of the event with an estimated repair costs exceeding $25,000 and was given nrc report #1177559. Swg provided an update to the nrc on may 6, 2017 at 15:55 and was given nrc report #1177639. Swg later received an e-mail that a correction was made to nrc report #1177639 and was issued nrc report #1177642.
