Narrative
On december 8, 2016, at 0733 hours, pg&e was notified of a third party dig-in near 4095 n golden state blvd in fresno. The dig-in occurred at the ashland ave on-ramp. Third party contractor, mcm construction struck a 2" plastic main while performing horizontal drilling. There was a valid USA ticket (x 631300829) and pg&e facilities were properly located and marked per pg&e procedures and agreement with the contactor; the contractor failed to excavate within a safe distance to protect the gas main from damage. Pg&e's gsr arrived on site at approximately 0815 hours. Pg&e repair crews arrived on site and shut in the flow of gas at 0917 hours. The repair crew installed a dead end as a temporary repair. 310 feet of new 2-in pl was installed; connected to 267 feet of existing 2-in pl and the service line; and all pipe was tested before returning to operations on 10/3/2019. One commercial trucking company customer service (4011 w ashlan ave, fresno) was impacted. Cng was set up and the customer was restored on 12/9/16 at 1000 hours. Police and fire department were reported on site. No known injuries, fatalities, or ignition have been reported with this incident. This incident is being reported to the cpuc due to the unintentional release of gas and presence of major media. After further review, estimated costs for repairs are expected to be greater than $50,000. The estimated repair costs were determined 12/9/16 at approximately 0900 hours, and reported to the DOT on 12/9/16 at 0939 hours.
