The leak was determined to be at a one inch (1") elbow weld inside the facility property. The root cause, as determined by metallurgical, analysis is attributed to fatigue of a crack feature that may have been present at the time of installation. As a corrective action, a portion of like pieces of equipment will be inspected using ndt to determine if there is a systemic issue. 12/6/2016 updated gps coordinates to reflect exact location of incident.
