A pinhole release occurred while welding the end weld of a type b sleeve during a rehabilitation dig. This line was shut down and isolated. Stopples were set and the failed pipe cut out and replaced with new, pre-tested pipe, and the line was subsequently returned to service. Approximately 400 cubic yards of impacted soil was land applied. This supplemental-final report was filed to list changes in costs in part d, and a changes requested by PHMSA accident coordinator.
