Incident originally reported to the nrc as 77 strawflower. Actual address of explosion is 91 strawflower ct. A third party excavator was installing conduit via directional drilling. A main was struck multiple times during the pullback. The release of gas was secured about 2 hours prior to the incident. Subsequently, the explosion occurred. In section g3 question 14, only one root cause can be selected; however, our investigation indicated 2 root causes; locating practices insufficient and excavation practices insufficient.
