Narrative
On july 8, 2016, los angeles fire department reported a structure fire at 4649 farmdale ave in los angeles. Crew responded and controlled the gas. One fatality was reported from this incident. Investigation into the cause of death and the cause of fire are ongoing. Additionally, it has not been determined whether the incident resulted from the release of gas or that the gas was caused by the incident. This incident was reported to cpuc and DOT due to the associated damage exceeding $50,000. A supplemental report will be submitted once the investigation has been completed. Updated information 6-16-17: investigation showed that gas was the not the primary cause of the structure fire.
