Narrative
On 3/9/2016 a fire was reported at the belle hollow station. Mountaineer arrived at approximately 07:30 and turned off gas station at approximately 07:35. Columbia gas transmission and energy corp. Of america also responded to shut off their facilities. The station was completely destroyed by fire. Investigation of cause is still underway. Mountaineer continues to share information and cooperate with the wv public service commission office of pipeline safety to complete the investigation. Added 5/9/2016: mountaineer has completed its investigation of the incident. Mountaineer continues to work with the wv public service commission-pipeline safety to do further evaluation on some equipment but those evaluations are not expected to be related to the incident. It is mountaineer's finding that the cause of the incident was a leak on the inlet/supply piping of the point of delivery station. This portion of the station was not owned or operated by mountaineer. Mountaineer has evaluated the condition of its remaining piping and equipment and determined that there was no failure of its piping or equipment other than secondary damage as a result of the fire.

Narrative
