Approximately 2 bbls of crude oil were released as a result of two failed insulator gaskets. The cause of the release was determined to be improper installation during construction. Both gaskets were replaced with a different type of gasket and verified for proper working condition. In addition, plains operators verified all gaskets were properly torqued. Furthermore, plains created a notification and awareness report (nar), to be sent to all relevant employees in order to help prevent reoccurrence.
