Approximately 25 bbls of crude oil were released as a result of a failure of the grease fitting on an aboveground ball valve. The grease fitting was replaced with a nipple, valve and plug. Supplemental narrative: part c, number 2 has been updated from 'onshore pump/meter station equipment and piping' to 'onshore terminal/tank farm equipment and piping' following an internal annual review.
