Narrative
On june 11, 2015 a rural farm tap meter setting was accidentally damaged by an agricultural sprayer. The sprayer sheared off a steel pipe nipple where it was threaded into the regulator. The damage to this remote meter setting and subsequent release of gas were not reported to columbia until july 4, 2015. Columbia personnel promptly responded and made the situation safe on july 4, 2015. On october 30, 2015, columbia engineers performed the gas loss calculation and determined that this event met the reporting criteria as set forth in cfr 49 part 191.3.
