Narrative
Ong's contractor damaged its 2" line while excavating to access and stop gas flow through a 2" gate valve, as part of a regulator station rebuild project. The valve could not be accessed using the valve chamber due to debris inside. Excavation work was performed to access the valve located at an approximate depth of 12.5 feet. Limited mobility in the work area, along with substantial rock, cement, and brick present in the excavation area complicated the excavator and spotter's efforts to properly locate and expose the valve.
