A contractor operating a grader failed to observe facility location marks and punctured an approximately 22 square inch hole in a 12 inch steel main. It is our understanding the contractor was under the impression the 12 inch steel main was relocated as a result of a recent main relocation project. The construction plans did not indicate the 12 inch steel main was to be relocated. Emergency responders were on-site and assisted in traffic control. Gas was made safe by closing a 6 inch valve on church road, north of the damage, stoppling a 6 inch three-way tee, northeast of damage at pleasant valley road and liberty parkway, closing the outlet valve of regulator station #007, south of damage at highway 69 and squeezing off a 6 inch plastic main on pleasant valley road, west of church road. A 30 inch long full circumferential band and a 48 inch long sleeve were welded over the damaged section of pipe. Repairs were completed at 16:27 on july 6, 2016, and service tech began relighting 210 customers who lost service.
