Narrative
A 4-inch steel main was hit and punctured by an excavating contractor's bore machine back-reamer while installing fiber optic facilities. The excavating contractor hand dug on both sides of the locate marking and could not locate the gas main. The locate contractor had not been able to correctly locate the facility. The company welded one short stop on each end of the damage and bypassed the main while the repair was made. Four feet of pre-tested steel pipe was attached to the existing main with two 4-inch posi-hold couplings. The company estimated repair costs to be below $50,000 and did not anticipate losses to be over three million cubic feet (mmcf); however, gas loss calculations conducted on march 19, 2015 totaled 10.873 mmcf. The company reported the incident to the national response center immediately after reviewing the gas loss calculations on march 23, 2015.
